Drinking Driver Program Instructor Application 
DPR-102  On-line fill-in form		 Used to apply to become a Drinking Driver Program instructor.  A separate form DPR-102 must be completed for each instructor. 2 pages
Drinking Driver Program Application
DPR-103  On-line fill-in form		 Application by a sponsor agency to establish a Drinking Driver Program under Article 31 of the NYS Vehicle and Traffic Law.

A resume for the DDP Director must be included with the form DPR-103.  The resume must include the name, address, telephone number, DMV Client ID Number from the driver license if applicable, the educational background and the employment background of the Director.

The applicant must also submit form DPR-104 and a form DPR-102 for each instructor.		 2 pages
Drinking Driver Program Service Agreement
DPR-104  On-line fill-in form		 All Drinking Driver Programs must agree to the terms included in form DPR-104 in order to operate a DDP.  The Service Agreement defines the responsibilities of the DDP to the motorist, to the program, and to the DMV.  Form DPR-104 is required to be submitted with form DPR-103 and all original application materials.  The applicant must also submit a form DPR-102 for each instructor. 2 pages
Drinking Driver Program Classroom Site Inspection Report
DPR-125  On-line fill-in form		 All Drinking Driver Programs must submit this form with DDP section completed for DMV to inspect the proposed classroom site. 1 page
DDP Fiscal Report
DPR-151 On-line fill-in form		 Required fiscal report to be completed annually by a Drinking Driver Program.

Consent for Release of Information/Withdrawal of Consent
DS-451  On-line fill-in form		 Consent and consent withdrawal form provided to DDP participants by the DDP. 2 pages
NYS Alcohol / Drug Rehabilitation Program Drinking Driver Program (DDP) as a Court-Ordered Conditional Discharge
DS-456		 Information for DDP program administrators and local courts about the option for a court to impose participation in a DDP as a conditional discharge in DWI or DWAI cases. 2 pages
Note:  There are currently two lists of service providers. Responsibility for the list of DWI Screening, Evaluation and Treatment professionals has been transferred to the NYS Office of Alcoholism and Substance Abuse Services (OASAS).  The Department of Motor Vehicles (DMV) continues to maintain the list of Drinking Driver Programs (DDPs).
